Diamond Willow has Provided Premier Memory Care & Assisted Living Services in Minnesota since 2004. To meet the requirements of our senior clients, we offer all-inclusive services ranging from feeding assistance, and daily activities to medication administration and continence care. In addition with the help of an individualized person centered care planning, we're able to tailor our care services to our patients' specific needs.
For an all-inclusive care delivery, we also offer coordination of services with hospice, home care, physical, occupational, and speech therapy. Besides, with weekly salon services, easy virtual visits, and above all 24/7 staff trained in memory care and personal care, we ensure that our client's needs are properly met. Here at Diamond Willow we take pride in our person centered care model which is uniquely different from most diagnosis based care systems.
With that comes the belief that bigger is not always better. Each setting is designed to be intimate with fewer residents per suite, allowing us to have smaller communities to promote our person centered care model in a truly calm and comforting home-like environment.

Diamond Willow was created as a bridge between traditional assisted living and a traditional nursing home.
Since that time we have modified our model to allow our elderly clients to live somewhere that can be considered home through the end of life.
We offer small intimate community settings of 8-16 residents per home and site based RN's who work with the onsite leadership team and the direct care staff to assure that your loved one's needs are being met.
Commitment: We are committed to providing quality service & care to our clients, through compassionate care completed in an intelligent, outcome based manner.

Diamond Willow Memory Care and Assisted Living is generally appropriate for anyone whose memory, safety, security, comfort or future well-being is compromised by remaining where they are, but who does not require the more intensive services offered in a nursing home.
This includes, but is not limited to people who are feeling uncomfortable living on their own, have a poor appetite, have trouble with medications (taking, not taking, over medicating, not taking at appropriate times, etc.), have increased forgetfulness, are dependent on an outside source for day-to-day needs, lack opportunities to socialize, cannot keep up with general housekeeping or maintenance, need transportation assistance or may need help with bathing or hygiene.
