The Assisted Living Program (ALP) is an Adult Home that is certified by the New York State Department of Health to provide Medicaid-funded home care services.
The ALP is an alternative to nursing home care that enables nursing home eligible people to receive home health care services in the Adult Home, delaying or preventing the need for admission to a nursing home.
Care services are provided by an on-site licensed home care agency operated by the Adult Home and a community certified home health agency (CHHA) under contract.
