We are a provider of the Assisted Living Waiver Program for both Assisted Living and Memory Care.

Must have Medi-Cal with NO share of cost.
Have a medical need that Assisted Living options would be able to help with.
If there is a diagnosis of Mild Cognitive Impairment (MCI) or Dementia- there must be a Power of Attorney (POA) of record.
