Almost 35 years ago our founder Sheny Graham opened her first 2-bed assisted living facility in her own home in Long Island, New York. There she used her years of care giving experience to provide a loving home for elderly clients, as she simultaneously raised her own kids in this same home. A couple of years later, she was able to buy a facility in Jacksonville, Florida, and the rest is history.
Services
Her heart assisted living employs a variety of positions, including: nurses, caregivers, CNA's, med techs, housekeepers, administrative/clerical staff, cooks, and kitchen assistants.

Fort Caroline Gardens is a 78 bed facility located in a nice and quiet residential neighborhood on Fort Caroline Road.
Although we have many residents who are healthy, alert, and independent, our staff experience allows us to provide a home where clients can age in place for many years to come.
We have a team of staff experienced and trained in providing a high level of assistance with all activities of daily living and also caring for those with dementia and Alzheimer's.
We have nurses on site seven days a week.

Autumn Village is an 80 bed facility located in the heart of historic riverside, three blocks away from St Vincent's hospital.
We have a beautiful secure and large courtyard with beautiful landscaping.
Many of our clients enjoy the flexibility and freedom of being able to walk in the riverside area, visiting the shops and cafe's in riverside.
